About the program

The Diabetes Clinic program supports people living with diabetes to feel more confident, informed, and in control of their health. Delivered by a qualified Diabetes Educator, the program provides personalised education and practical coaching tailored to each individual’s needs, goals, and lifestyle.

Participants receive one on one support to better understand their condition, develop a personalised care plan, and build everyday skills for managing diabetes. This may include support with blood glucose monitoring, nutrition and activity planning, medication support as appropriate, recognising and responding to highs and lows, and setting achievable health goals. The program focuses on sustainable, person centred care and works alongside existing healthcare providers to strengthen ongoing self management and wellbeing.

Supports available

We offer a range of specialised services to support individuals living with chronic health conditions:

  • Diabetes education
  • Dietetic support
  • Care Coordination

Our care coordinators will work with you to assist you manage your chronic health conditions through the development of individual plans and increasing self- management capacity. 

Referral pathways

Our services are designed to support individuals living in the Peel region of WA who are over 18 years old and managing diabetes. We prioritise providing care to those who face financial disadvantage or have limited access to essential health services.

By offering tailored support, we help improve health outcomes and bridge gaps in access to quality healthcare.

Individuals can be referred for our services by their GP, Nurse Practitioner or the Remote Area Nurse.  Referrals must include a current copy of the GP Care Plan and the reason for the referral.

The ICDC service is provided through funding from WAPHA
The WB Diabetes Education service is provided through funding from WACHS
