About the program
In our Aftercare Services Program (ASP), our lived experience- led team provides free support for up to 90 days, for people who have experienced a suicidal crisis and are leaving hospital.
This is not an emergency service. If you are in crisis, call 000 or go to the nearest emergency department. We work with your mental health team and community supports to help you stay connected.
Supports available
The Aftercare Services Program (ASP) works closely with community mental health services, GPs and other service providers.
Our services provide practical and emotional support tailored to your needs, including:
- Person‑centred support tailored to the individual’s needs
- Safety planning and managing triggers or warning signs
- Practical help with housing, daily living, appointments, and navigating services
- Emotional and therapeutic support through counselling or psychosocial interventions
- Connection to community supports like GP, psychologist, psychiatrist, family, and peer networks
- Multidisciplinary approach– a mix of peer support and clinical workers
- Recovery focus, helping the person build stability, confidence, and wellbeing
Referral pathways
You may be eligible for the Aftercare Servicess Program if you:
- Are aged 16 years or over
- Have presented to Bunbury Hospital or Busselton Health Campus (or another South West Emergency Department) due to suicidal thoughts, behaviours, or crisis
- Are being discharged from hospital and assessed as safe to receive non-clinical support in a community primary health care setting.
External referral pathways are not available for this program, referrals are limited to Busselton & Bunbury Emergency Department, approved referrers.
