About the program
My Health Partnerships is a pilot program offering clinical care coordination for those with, or at risk of, chronic physical or mental health conditions.
Care Coordinators identify health needs, connect individuals to appropriate services, and provide advocacy and support. They
collaborate with other providers and facilitate a monthly Outreach Hub for outlying communities in the Warren Blackwood region.
Supports available
Our care coordinators are dedicated to supporting clients in managing their health and wellbeing by providing tailored assistance and guidance.
- Identifying health and wellbeing needs
- Connecting clients to appropriate services
- Navigating complex health systems
- Coordinating follow-up to ensure effective communication among providers
Through these services, we help ensure clients receive the support they need to address their health challenges and achieve better outcomes.
Referral pathways
Our services are available for people aged 16 years and older, who live in or use health services within the Warren Blackwood region.
We aim to promote equitable access to health and wellbeing services for all eligible members of our community.
You can access My Health Partnerships through:
- Walk-ins at our Manjimup Health Hub or one of our Outreach Hub centres
- Self-referral via phone or email
- Referrals are welcomed & accepted from
health and community organisations
