About the program

My Health Partnerships was a pilot program funded the under Innovative Models of Care (IMOC) Program. The program offered clinical care coordination for those with, or at risk of, chronic physical or mental health conditions.

Care Coordinators identify health needs, connect individuals to appropriate services, and provide advocacy and support. They
collaborate with other providers and facilitate a monthly Outreach Hub for outlying communities in the Warren Blackwood region.

Supports available

Our care coordinators are dedicated to supporting clients in managing their health and wellbeing by providing tailored assistance and guidance.

  • Identifying health and wellbeing needs
  • Connecting clients to appropriate services
  • Navigating complex health systems
  • Coordinating follow-up to ensure effective communication among providers

Through these services, we help ensure clients receive the support they need to address their health challenges and achieve better outcomes.

Referral pathways

Trial activities commenced in September 2022 and concluded in January 2026. The final report and model of care are currently under development.
