About the program

The Diabetes Education program provides personalised education, practical support, and self-management strategies to help people better understand and manage their health.

Our Diabetes Educators work with individuals to build confidence, improve health outcomes, and develop skills for living well with diabetes and other chronic conditions.

Supports available

The following services are available for adults:

  • Type 1 Diabetes support
  • Type 2 Diabetes support
  • Newly diagnosed diabetes education
  • Insulin initiation support
  • Continuous Glucose Monitoring (CGM) initiation
  • Pre-diabetes education and prevention support
    Diabetes self-management coaching
Please note: GP referrals are required. GP Management Plans are accepted
Referral pathways

Our services are designed to support individuals living in the Peel region of WA who are over 18 years old.

By offering tailored support, we help improve health outcomes and bridge gaps in access to quality healthcare.

Individuals can be referred for our services by their GP.  Referrals must include a current copy of the GP Care Plan and the reason for the referral.
