About the program

Oseca Health [Oseca] is the lead agency of the Peel Health Hub (PHH), a central location for the health needs of young people,
individuals, families, and carers in the Peel region.

Our friendly team of qualified professionals provides a safe, confidential point of contact, helping to create care plans, initiate treatment, and connect clients with both internal
and external services.

Supports available

Oseca provides a range of services at the Peel Health Hub, focusing on coordinated care and support for individuals who may not know what services they need. Supported by GPs and nursing staff, the Clinical Care Coordination (CCC) model ensures clients receive personalised, seamless care to improve their health and wellbeing.

Services include:

  • GP & Nursing services (limited)
  • Clinical Care Coordination (CCC)
  • Personalised care planning
  • Access to health and welfare services within the Peel Health Hub
  • Assistance with accessing external services
  • Safe and confidential first point of contact
  • Initiation of treatment with client consent
Referral pathways

Our Oseca PHH team provides primary healthcare and advanced clinical access interventions to young people and families in the Peel Region. This ensures early comprehensive assessment, initial care-planning and timely care.

We operate a ‘no wrong door’ policy, so if you or someone you  know needs help please contact us by phone, email or in person

Referrers (inclusive of external GPs) should contact the Clinical Care Coordinator triage officer at PHH, to discuss potential referrals in the first instance. 

Please note the Peel Health Hub does not provide 24 hour crisis support
